If the left green arrow burns out the cross street doghouse can still light the green arrow, its on the same switchpack. If the yellow burns out on the left turn signal (assuming the clearance yellow in a FYA setup), the doghouse yellow right turn arrow will still light up, but its not on the same switch.
Here is an example of how these are connected. In the controller, selecting a movement to be a flashing yellow arrow on the EPAC300's requires you to go into Unit Data> overlap special, and select the overlap you want and then the phases in which the protected movements will operate and the permissive phases that will be green overlapping a flashing green interval (the flashing yellow arrow)

I believe the unused red output is tied to AC hot, that is how I have it in my cabinet, but not sure if that's how they take care of it in the real world
Also in the diagram the two signals represented would be on crossing streets not on the same approach
